The process is led by children’s nurses based within a children & young people continuing care team.
A continuing care package will be required when a child or young person has needs arising from -
- disability
- accident
- illness
that cannot be met by existing universal or specialist services alone.
Referrals can be made by -
- parents
- carers
- health professionals
- social care and voluntary section
