The Child Disability Team provides specialised support for children and young people with moderate to severe disabilities, offering a range of services to assist families and promote development.

What is the Child Disability Team?

The Child Disability Team is a specialised group within social services that focuses on the needs of children and young people with disabilities. This team typically includes social workers, SEND disability support workers and paediatric occupational therapists who collaborate with various professionals, such as health visitors and occupational therapists, to provide comprehensive support.

It also offers leisure and freetime support services (LAFSS) for children and young people from 0-25 years.

Services Offered

The Child Disability Team offers a variety of services aimed at supporting the development of disabled children and assisting their families. These services may include:

  • Assessment and Support: Evaluating the specific needs of children with disabilities and providing tailored support plans. Short break assessments.
  • Information and Advice: Offering guidance to families about available resources and services.
  • Collaboration with other professionals and voluntary services: Working alongside healthcare providers, educators, and therapists to ensure a holistic approach to care.

How to access the Child Disability Team

You can make a request by contacting the Early Help and Safeguarding Hub (EHASH) by completing the form below. If you are unable to use the portal, you can complete a request for service form. This can be emailed to EHASH@hullcc.gov.uk.

EHASH will triage all requests. If a referral is made to the Child Disability Team from EHASH a member of the team will contact you.
